Client Consent Forms

ALL patients must have a complete and signed ‘Surgical Consent Form’ prior to scheduling surgery.

Clients with patient breeds prone to airway abnormalities (English Bulldog, French Bulldog, Pug, Shih Tzu, Boston Terrier etc) must ALSO complete and sign the ‘Brachycephalic Consent Form’.
